Your first appointment

Before your first appointment with Dr O’Leary, you will be asked to complete a new patient registration form and a foot and ankle scoring questionnaire. Links to the forms will be provided when you have a confirmed appointment.

Remember to bring your:

  • Referral letter from your GP or other medical specialist
  • Medicare card, DVA card, pension card
  • Private health fund card or details
  • Any relevant previous imaging (X rays, CT scans, MRIs etc)
  • Any other relevant information, such as a list of your current medications
  • Completed patient registration form and scoring questionnaire

If you need to cancel or postpone your appointment

We understand that you may need to cancel or postpone your appointment, sometimes at short notice. If this is the case, please call during business hours, at least 24 hours before your appointment, so that we can offer it to a patient on our waiting list.
